2 Wellness Visits, each of which includes:

  • Dental exam
  • Dental cleaning
  • Oral cancer screening
  • Periodontal evaluation
  • Oral health instruction
  • Fluoride Treatment
  • All necessary x-rays

All Emergency Exams, which include:

  • The doctor will evaluate your concerns and present treatment options.
  • Diagnostic tests, such as an x-ray are included with your exam to help make a diagnosis and get you back on track.

This is not an insurance plan but rather a dental care membership program that guarantees you a specific amount of preventive services as well as a discount on most dental procedures.

Excluded services include Invisalign orthodontic cases. THis plan is available at our Eagle Rd. location only and does not pertain to referred services outside our office.

This plan cannot be used for the following:

  • In conjunction with another dental plan.
  • For services or injuries covered under workman’s compensation.
  • For referrals or lab costs.
  • For hospitalization of any kind.
